29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous.    a) West end of Bay 10: A TPI industrial floor fan was equipped with a 120 volt electrical cord, that was missing the ground prong.  b) Northwest end of Bay 10: A 120 volt electrical extension cord used to power the TPI floor fan, was missing the ground prong.  MICHELL BROTHERS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION 301113783, CITATION 1, ITEM 1, ISSUED ON 09-23-2009.
