29 CFR 1910.151(a):     Medical personnel were not readily available for advice and consultation on matters of plant health:    On or about July 17, 2012 and times prior thereto, a temporary employee of Welspun Tubular, LLC performed jeeping operations on steel pipe with the use of a Spy� in-plant holiday detector system model 115 in the Coating Department located at or near 9301 Frazier Pike in Little Rock, Arkansas.  The employee had suffered from an electric shock in the process of grounding the pipe which was reported to the employer.  The employer had not followed their Emergency/Incident Policy and Procedures for incidents and accidents and medical personnel were not available for advice and consultation on the matter of the electric shock.

29 CFR 1910.335(a)(1)(i):     Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    On or about July 17, 2012 and times prior thereto, temporary employees of Welspun Tubular, LLC performed jeeping operations on steel pipe with the use of a Spy� in-plant holiday detector system model 115 in the Coating Department located at or near 9301 Frazier Pike in Little Rock, Arkansas.  Employees performing this operation were exposed to the hazards of electric shock in the event the test electrode or ground was touched while the Spy� in-plant holiday detector system was activated.  The employees were not provided with electrical protective gloves appropriate for the work performed.
