29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a.  Planing Mill, Trim Saw - On 08/24/2012, accumulations of explosive wood dust were not adequately controlled. Accumulations were observed on the trim saw enclosure, overhead lighting, cable tray, rafters, & crossmembers on the walls. The wood dust on the trimsaw enclosure was approximately 0.5 - 4 inches deep.

29 CFR 1910.265(c)(20)(ii):     Machine(s) that created dust, shavings, chips, or slivers during a period of time equal to or greater than one-fourth of the working day were not equipped with collecting system(s) of sufficient strength and capacity to enable the removal of refuse from points of operation and immediate vicinities of machines and work areas:    a.  Planing Mill, Trim Saw - On 08/24/2012 the trim saw line was not equipped with a collection system of sufficient strength to prevent the escape of sawdust from the saw enclosure. Sawdust had accumulated on top of the enclosure at depths of about 0.5 - 4 inches.     b.  Sawmill, Brewer Resaw - As of 08/24/2012 the collection system for the Brewer resaw was not operating with sufficient strength and/or capacity to remove sawdust generated by the resaw. Dust had accumulated to approximately 0.25 inches in 2 hours on the floor. Sawdust accumulations generated over several days up to about 6 inches were found on the machine frame.

Sawmill, Cant Deck - As of 08/24/2012, appropriate head protection was not used by the Brewer resaw operator who could be struck by cants being transported to the resaw while he was turning boards on the lumber deck.

Wood Boiler Room - Two employees, who use tight fitting full facepiece respirators, had not been fit tested since 03/26/2010.   During the time period between 05/10/2012 to 05/17/2012, the employees performed jobs, such as the clean-out of the wood fired boiler and inspection of the silo, both of which required the use of these respirators.

29 CFR 1910.265(c)(4)(ii):     Walkway(s) were not evenly floored:    a.  Resaw - As of 08/24/2012, a section of the walkways on either side of the Cant/Lumber conveyor decks were unevenly floored. There was a change in the elevation of the walkway. The sections had a step down onto a side-sloped surface. The slopes were approximately 10 & 15 degrees toward the wall.

29 CFR 1910.303(b)(7)(iv):     There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:    a.  Sawmill,  under the metal detector for the chipper - On 08/24/2012 the cover on an electrical  junction box had been punctured permitting entry of sawdust through the opening.  Additionally, the conduit running into the side of the junction box had loosened creating a space which also would allow the entry of sawdust.    b.  Sawmill, pit below trim saw takeoff - On 07/16/2012 the corners of the cover on an electrical junction box had been bent back and the sealing layer had loosened.
