29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets On or about April 13, 2012, employees performing roofing activities at a height over 16 feet were not protected from falling. Employees were exposed to an unprotected fall to the concrete below.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. On or about April 13, 2012 and at times prior thereto, at a job site located at the Tarrant County College North East Campus, North Richland Hills, Texas; at least two employees were exposed to a fall and struck-by hazard while working next to and/or standing on a pallet elevated on the forks of a lift truck that was left unattended in a raised position.
