29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 6940 NE 69th Terrace, KC MO -- employees engaged in roofing duties were not wearing eye protection while operating pneumatic nail guns and staplers. This exposed employees to getting particles of debris in their eyes. This was documented on or about 05/03/2021.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a residential worksite located at 6940 NE 69th Terrace, KC MO -- Employees were engaged in roofing duties with any fall protection system in place. This exposed employees to fall hazards of 17 plus feet to ground level.  This was documented on or about 05/03/2021.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential worksite located at 6940 NE 69th Terrace, KC MO -- The ladder used by employees to gain access to the roof to perform roofing duties did not extend at least three (3) feet above the landing surface of the roof. This exposed employees to fall hazards of 15 feet to ground level from the roof.
