29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. In the welding area, a compressed gas cylinder was not secured to prevent it from being knocked over. An employee was exposed to struck-by hazards.

29 CFR 1910.1026(d)(1): Each employer who has a workplace or work operation covered by this section shall determine the 8-hour TWA exposure for each employee exposed to chromium (VI). This determination shall be made in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section. An initial assessment of welders' 8-hour time-weighted exposure to hexavalent chromium was not conducted for the chrome carbide and stainless steel bucket welding operations. The employees were exposed to health hazards associated with hexavalent chromium exposure.

29 CFR 1910.1026(l)(1): General. In addition to the requirements of the Hazard Communication Standard, 29 CFR 1910.1200, employers shall comply with the requirements in 29 CFR 1910.1026(l)(2)(i) - (ii): The employees welding on stainless steel material were not trained on the health hazards, location and use of chromium compounds, and protective measures needed against exposure to hexavalent chromium.

29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer. The information contained in Appendix D of the respirator standard was not provided to employees when voluntary use of 3M filtering facepiece respirators was permitted by the employer.
