29 CFR 1926.502(h)(1): The employer did not did not designate a competent person to monitor the safety of other employees and/or did not ensure that the safety monitor complied with the requirements listed in (h)(1)(i) through (h)(1)(v) of this section:    a. On or about July 3, 2012, the employer did not designate a safety monitor for employees working on a low sloped roof outside the warning lines.  The employees were exposed to 20 feet fall hazards.     Abatement Certification and Documentation are not required

On or about July 3, 2012, the ladder placed on the stair case was not secured to prevent accidental displacement nor was there a barricade used to keep activities away from the ladder.
