OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to impalement hazards while walking and working near steel rods protruding up from the concrete surface. (a) Location - Column line C4: On 06/29/12, prior to commencement of erecting steel columns, employees were exposed to impalement hazards from 1" and 1/4" diameter threaded anchor rods protruding from the concrete floor. (b) Location - South wall area: On 06/29/12, employees were exposed to impalement hazards from a 1/2" diameter copper clad lightning / ground rod protruding up from the concrete surface.

(b) Location - 47 Harvard Way, Allston; 17 line -west area way: On 06/29/12, an employee working on forms was exposed to impalement hazards from unprotected rebar ends protruding upward.
