Jobsite: Employees were allowed to use an alloy steel-chain sling to list a load/MEGA Brace System that was in excess of the rated capacity working load limit of the sling.    On August 14, 2014, the steel-chain sling failed during the process of lifting the load, and one of employees was hit by the lifting equipment/come-a-long. The employee sustained serious injuries from the incident.
