29 CFR 1910.216(a)(4): New mill installations were not installed so that the top of the operating rolls was not less than 50 inches above the level where the operator was standing, irrespective of the size of the mill: (a) Rubber Building Area - The top of the operating rolls on the Cracker had a height above the operator's platform of 46 inches, putting the operator in closer proximity to the bite than permitted by the standard, on or about July 18, 2012. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and shall state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: (a) Maine Industrial Tire, LLC, 207 Redco Avenue, Red Lion, Pennsylvania - The employer did not ensure employees were trained in first aid administration when medical response time provisions exceeded four minutes from the time of the accident occurrence to the treatment of the injured employee, on or about July 18, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and shall state that employees and their representatives were informed of this abatement.
