29 CFR 1926.405(g)(2)(iv):   Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about July 20, 2012, an employee plugged in an extension cord to power the portable drill battery charger where the end of the cord was not equipped with strain relief and the insulated wires were exposed, exposing the employee to an electrical shock hazard.
