29 CFR 1910.133(a)(1):     The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a) U.S. Department of Veterans Affair, Decatur, GA - In the Canteen Kitchen, an employee was transferring Ecotemp liquid soap, classified as corrosive, from one container to another and connecting the container to the dishwasher without using eye protection in the event of a splash, on or about 7/18/12.    b) U.S. Department of Veterans Affair, Decatur, GA - In the Histology Lab, employees added formalin to containers with tissues received from the operating rooms. The use of splash resistant goggles was not ensured, on or about 7/20/12.    c)  U.S. Department of Veterans Affair, Decatur, GA - In Food and Nutrition Department, employees handled Grill Brite, a grill and oven cleaner, classified as corrosive, without using eye protection in the event of a splash, on or about 7/20/12.    d) U.S. Department of Veterans Affair, Decatur, GA - In Food and Nutrition Department, employees poured Oasis 115XP, classified as corrosive, into the floor scrubber,  without using eye protection in the event of a splash, on or about 7/20/12.
