29 CFR 1910.178(l)(4)(ii)(E):  Refresher training in Powered Industrial Truck operations was not provided to the operator when a condition in the workplace changed in a manner that could affect safe operation of the truck:    a) ASA Apple Inc. - Carteret, NJ site - Warehouse: ASA Apple Inc moved from a storage facility located at 100 Washington Avenue, Carteret, NJ to the current facility at 377 Roosevelt Avenue, Carteret, NJ.  The facility has not completed the required forklift re-training based on the change of work conditions by moving from one facility to another.    Condition observed on or about July 23. 2012.
