On or about July 23, 2012 the employer did not provide appropriate eye protection for employees while they were exposed to frozen tissue hazards during the changing of liquid petroleum tanks on forklifts. b) Planet Aid Inc. On or about July 23, 2012 the employer did not provide appropriate eye protection for employees while they were exposed to eye penetration hazards associated with bailing wire.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: a) Planet Aid Inc. Bric & Brac area, warehouse - On or about July 23, 2012, metal Knock-Out box type receptacles with light switches attached were dislodged from the wall anchors and conduit sections allowing for the inner wires to be exposed and subjected to abrasions. b) Planet Aid Inc. Bric & Brac area, warehouse - On or about July 23, 2012, lengths of conduit connected to light switches were separated and dislodged from the wall anchors allowing for the inner wires to be exposed and subjected to abrasions.
