29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees in the warehouse operating and working near hydraulic balers were exposed to crushing hazards in that the balers were operated with the gates not fully closed in the following instances: A) The hydraulic baler on the east side of the warehouse had a gate guard that did not close all the way, leaving an unguarded gap of 1 inch between the gate door and the baler when the baler was operating. B) The hydraulic baler on the west side of the warehouse had a gate guard that did not close all the way, leaving an unguarded gap of 3 inches between the gate door and the baler when the baler was operating. Planet Aid Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1050191, citation number 1, Item number 2 and was affirmed as a final order on June 11, 2015, with respect to a workplace located at 47 Sumner Street, Milford, MA 01757.
