29 CFR 1910.22(a)(2):     The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.    a. White Room/ East Wall - On or about July 30, 2012, employees were required to work in an area where the metal grid on the drainage was not properly maintained. The conditions of the metal grid found were the following;    1. A portion of the metal grid of 1 1/2" lenght by 4" wdth was broken.    2. A portion of the metal grid of 2" length by 4" width was broken.     3. Some other portions of the metal grid were removed from their locations and were placed on the floor or leaning against the wall.

29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a. West side of Blending Building - On or about July 23, 2012, employees were required to enter and leave the building through an outdoor walking platform at above 4 feet of the ground level. The walking platform was not guarded by a standard railing system on all open sides . Employees were thereby exposed to fall hazards.

29 CFR 1910.23(d)(1)(iii):     Every flight of stairs with more than 4 risers and at less than 44 inches wide shall be equipped with an standard railings on both open sides:      a. West side of Blending Building - On or about July 23, 2012, employees were required to enter and leave the building through an outdoor platform that had its staiway at less than 44 inches wide and was not equipped with railings on each side. Employees were thereby exposed to fall hazards.

29 CFR 1910.23(e)(1):     A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp.     a. West side of Blending Building - On or about July 23, 2012, employees were required to enter and leave the building from a 52 inches metal platform that had its guardrailing system missing their intermediate railings. Employees were thereby exposed to the hazards of falls.

[63 FR 33450, June 18, 1998]    a. Blending Building - On or about July 30, 2012, employees required to work with corrosive chemicals, such as but not limited to sodium hydroxide, were not provided with adequate eye wash stations. The defficiencies found regarding eye wash stations and their locations were the following;    1. The Pre - weight room did not have an eye wash station.    2. The White room had an eye wash station that was not operable (low water flow).

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    a.South Building Area/South side/Line #1/Exit Door # 3 - On or about July 23, 2012, the exit route of the building was obstructed with a wooden pallet containing carboard boxes. Employees were thereby potentially exposed to the hazard of fire.
