29 CFR 1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. Production Dept./North Line #5 - On or about August 8, 2012, the employer did not develop and document specific procedures to control and lockout all energy sources on the Anderson Work sorter. Employees were required to perform maintenance, unjam, and/or clean the machine, thereby employees were exposed to serious injuries.

29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). Production Dept./North Line #5 - On or about August 8, 2012, the employer did not ensure that an energy isolating device was available to control the energy sources on Anderson Works sorter when the employees service and/or unjam the caps from the sorter machine. Employees were thereby exposed to to the hazards associated with rotating moving parts.

29 CFR 1910.178(m)(10): A load backrest extension shall be used whenever necessary to minimize the possibility of the load or part of it from falling rearward. Entire Facility - On or about August 8, 2012, the Yale #4 forklift truck was operated with no load back rest extension. Employees were thereby exposed to falling materials.
