29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. Arc Light Blvd, Andersen Air Force Base, Yigo, Guam. On or about July 23, 2012 and at times prior thereto, the employer failed to provide an employee operating a Bosch Jackhammer with approved safety eye protection exposing the worker to serious eye injuries from cement and asphalt debris.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: Arc Light Blvd, Andersen Air Force Base, Yigo, Guam. On or about July 23, 2012 and at times prior thereto, the electrical plug on an extension cord connected to the Bosch Jackhammer was frayed and broken exposing electrical wiring and exposing workers using the jackhammer and other electrically powered tools to electrical injuries.
