29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, persoanl fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented. (a) Front Roof located at714 Park Manor Drive, Smyrna, GA 30080: Employees engaged in roofing activities on a 12/12 pitched roof with an eave height of approximately 15'4" were not provided with an adequate fall protection system. One employee had no fall protection system in place and three employees were equiped with positioning belts attached to a life-line.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the area specified in 29 CFR 1926.503(a)(2)(i) through (viii): (a) Roof located at 714 Park Manor Drive, Smyrna, GA 30080: The employer did not provide fall protection training to employees working on a 12/12 pitched roof at eave heights of approximately 15'4". Employees were wearing inadequate fall protection systems which included the use of positioning belts and one employee was provided no fall protection at all.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper surface to which the ladder was used to gain access. (a) Front side of residential home located at 714 Park Manor Drive, Smyrna, GA 30080: The Werner 28' aluminum ladder provided did not extend 3 feet above the working surface of the roof to allow for safe access on and off the roof.
