29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Site - On or about 02/22/18 an employee walked and transported shingles along the roof ridge without fall protective equipment, and was exposed to falls from a 25 ft maximum elevation to ground level. (Roof slope on ridge: 12:12).  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Site - On or about 02/22/18 an employee accessed the garage roof with an aluminum ladder which did not extend three feed above the landing surface. (Garage roof eave elevation: 11 ft.).  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1060(a)(1): The employer did not ensure that each employee had been trained by a competent person in the areas described in paragraph (a) of this section:  a) Site - On or 02/22/18 an employee accessed the garage roof with an aluminum ladder which did not extend three feet above the landing surface, and the employer did not ensure that training was provided by a competent person on the recognition of hazards associated with ladders.
