29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about July 25, 2012, the employer failed to ensure employees wore proper eye/face protection while cutting wood on a roof.  The employee was using a Craftsman 7 ? inch circular saw model number Z2018 780.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):      On or about July 25, 2012, the employer failed to ensure employees working from a 8/12 pitch roof were protected from a fall of approximately 13 feet 2 inches by using an adequate fall protective system/devices. Employees were performing tear off and replacement of weatherproofing from a residential roof.
