H-M Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1048980,citation number 1, item number 1, and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 5900 Hodgman, Parma, OH 44130.

H-M Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1048980,citation number 2, item number 1, and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 5900 Hodgman, Parma, OH 44130.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about May 16, 2018, during the installation of roofing materials on a residential home, the employer failed to ensure employees working at a height of approximately 19 feet wore harnesses in which the attachment point was located in the center of the wearers back. The employees were attached to a center point on their tool belts.   H-M Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(17), which was contained in OSHA inspection number 1048980,citation number 1, item number 2d, and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 5900 Hodgman, Parma, OH 44130.
