29 CFR 1910.303(b)(1):     Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  Location: Sitewide.  Employees were exposed to electrical hazards where a competent person had not taken appropriate steps to abate several unsafe conditions pertaining to electrical equipment throughout the building.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Jobsite: Several locations throughout building.  Employees were exposed to electric shock, arc flash/blast, and other electrical hazards while working on or in close proximity to electrical panels that were missing covers and inner bezels, exposing the live busbars and other live parts within.

Employees were exposed to electrical and fire hazards where:  a) Two instances of temporary wiring that had been left in place of a permanent installation where several feet of individual conductors were strung without being housed in a raceway.  b) conductor splices had not been placed in a junction box.

Employees were exposed to electrical shock and other hazards while working in and around electrical equipment where the branch circuit breakers in 208 volt, 3-phase panels had not been marked to indicate their purpose.

Jobsite: Site.  a) The plug on a fan attached to the wall in the kitchen was missing the safety grounding pin.  b) The plug on a pedestal fan in the main equipment room was missing the saftey grounding pin.
