29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. (a) Bruceton Mills, WV - Employees were working on scaffolding greater than ten feet off the ground around the outside of a sewage treatment tank under construction and the end of one of the scaffolds did not have a guardrail in place as determined on July 23, 2012.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect and a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) Bruceton Mills, WV - The step ladder used to gain access to a scaffold located inside of a wastewater tank under construction extended less than one foot above the platform accessed by employees without the ladder being tied off and no grabrail installed as determined on July 23, 2012.
