29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a) Facility Wide- The procedures supplied by the employer did not clearly and specifically outline the scope, purpose, authorization, rules and techniques outlined in A-D of this section to be utilized for the control of hazardous energy and the means to enforce compliance for equipment in the facility such as the Punch Presses and Press Brakes. The procedures failed to include the following: -Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. -Specific procedural steps for the placement and removal of lockout devices or tagout devices and the responsibility for them. - Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
