One (1) employee was observed using a 20-ft extension ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed;     b.) One (1) employee was observed using a 20-ft extension ladder, and the ladder's side rails did not extend at least 3 feet above the upper landing surface being accessed.
