29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a) On 07/26/2012, employer did not inform affected or authorized employees of the existence and location of grain silo permit required confined spaces by posting danger signs or equally effective means of training for confined space entrances located at the lower catwalks for the two most easterly silos outside the facility adjacent to the train tracks. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
