29 CFR 1910.212(a)(2): Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about April 27, 2017 the cover guard located at the bottom of the vertical auger on the feed truck was not secured in a manner that prevented easy access to auger equipment.

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented, and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. (a) On or about April 27, 2017 at the Srnka Farms LLC located at E4127 Pheasant Road, Algoma, Wisconsin, energy control procedures were not developed and utilized to identify the specific techniques required to perform cleaning activities in the Walinga trailer. This exposed employees to struck by and caught in hazards from the unguarded vertical auger. The CP Feeds LLC was previously cited for a violation of this occupational safety and health standard [29 CFR 1910.147(c)(4)(i)], which was contained in OSHA inspection number 543818, citation number 1, item number 1 and was affirmed as a final order on October 29, 2012, with respect to a workplace located at 16322 W. Washington St., Valders, WI 543818.
