29 CFR 1926.404(a)(2):     A grounded conductor was attached to a terminal or lead so as to reverse designated polarity:  (a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)    (a) Residential Project Chalan Inda Pago Bay Chalan Pago: On July 30, 2012 and at times prior thereto, outlets providing temporary power to the site through flexible cords were wired to a lead so as to reverse designated polarity.  Employees were exposed to electric shock and burns.

29 CFR 1926.404(b)(1)(i):     General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    (a) Residential Project Chalan Indu Pago Bay Chalan Pago: On July 30, 2012 and at times prior thereto, temporary power to the site was not equipped with ground fault circuit interrupters nor was an assured equipment grounding program in place at the site.  Employees were exposed to electric shock and burns.

29 CFR 1926.404(f)(6):     The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  (a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)    (a) Residential Project Chalan Inda Pago Bay Chalan Pago: On July 30, 2012 and at times prior thereto, the path to ground between the temporary power and the Rigid Hammer Drill used as a wet mortar mixer was not permanent and continuous.  The yellow extension cord transmitting power to the tool was missing its ground pin.  Employees were exposed to electric shock and burns.

29 CFR 1926.1052(c)(1)(ii):     Stairways having four or more risers or rising more than 30 inches (76 cm), whichever was less, were not equipped with one stairrail system along each unprotected side or edge:    (a) Residential Project Chalan Indu Pago Bay Chalan Pago interior main stairway: On July 30, 2012 and at times prior thereto the unprotected sides of the stairway to the second floor was not equipped with a stairrail system at the west side and east side of the lower stairway to the mid-level landing and the west side of the stairway above the mid-level landing.  Employees were exposed to falls a distance of up to ten feet, six inches to the concrete floor below.

J YONG CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314656752 CITATION NUMBER 1, ITEM NUMBER 2, ISSUED ON NOVEMBER 17, 2010 AND CONFIRMED AS A FINAL ORDER ON DECEMBER 10, 2010 WITH RESPECT TO A WORKPLACE LOCATED AT E.T. CALVO WAY, MAITE, GUAM.
