29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigi. a. At the job site - The ladder being used to access the roof was not erected properly. The side rails of the ladder only extended 12-inches above the upper landing, exposing employees to a 10.5-foot fall hazard, on or about July 18, 2012.
