29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):  On or about June 21, 2012:  Employee was exposed to fall hazard of approximately 12.3 feet while installing roof sheathing on a new home under construction located at 4860 Dumbbarton Ct, Cumming, GA.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 21, 2012:  Employer failed to implement and train employee in fall protection measures, which would have allowed employees to recognize and minimize fall hazards while being exposed to fall hazards of approximately 12.3 feet, while installing roof sheathing roof section of home at Lot 6 located in the Kingsworld at Castleberry Subdivison in Cumming , GA.
