29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    (a) Rear side of residential home and inside garage at 761 Park Manor Drive, Smyrna, GA 30080: Employees using a Makita 7 1/4" Circular Saw, Bostitch Pneumatic Nailer, and a Hitachi Pneumatic Siding Nailer were not protected from eye injuries while cutting and installing fascia boards.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, nor did it have guardrails which block employee access to the cantilevered end.                                                                                     (a) Rear side of the residential home located at 761 Park Manor Drive, Smyrna, GA 30080: Employee was exposed to a fall hazard while working on a ladder jack scaffold platform that extended approximately 6' past the support ladder jack which had no guardrail system to block employee's access to the cantilevered end. The work platform measured approximately 15'3" from the ground below and was not secured in any way to prevent tipping.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components were not either immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    (a) Rear side of home located at 761 Park Manor Drive, Smyrna, GA 30080: The 32' Green Bull Aluminum Extension Ladder used to support a ladder jack scaffold platform had multiple damaged rungs and a damaged locking latch and was not removed from service. Employee worked at heights of approximately 15'3" on a ladder jack scaffold platform supported by this damaged ladder.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than ten feet above a lower level was not protected from falling to a lower level:    (a) Rear Side of the house located at 761 Park Manor Drive, Smyrna, GA 30080: The employee installing fascia board from a working platform of a ladder jack scaffold, approximately 15'3" above ground level, was exposed to fall hazard, as the employee was not wearing a personal fall arrest system and neither guard rails nor safety nets had been installed.      Samuel Quintanar was previously cited for serious violations of the Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.451(g)(1) which was contained in OSHA Inspection Number 314643339, Citation #1, Item #2, issued on 07/25/2011 with respect to a work place located at 12552 Dunbrody Avenue, Alpharetta, GA 30004 and became a final order on 08/23/2011.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided:    (a) Rear side of residential home located at 761 Park Manor Drive, Smyrna, GA 30080: An employee was exposed to a fall hazard of approximately 10' while installing fascia board on a 4/12 shed style roof with no fall protection system in place.    Samuel Quintanar, was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 314641697, Citation #1, Item #1, issued on 07/12/2011 with respect to a work place located at 12512 Broadwell Road, Alpharetta, GA 30004 and became a final order on 08/17/2011.
