29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  (a) Temporary electric service, new residential construction located at 748 Manor Drive, Smyrna, GA 30082: Employees used a 100' black and red extension cord with a missing ground pin to power a Porter Cable (Model C2002) 150PSI, 6 Gallon Air Compressor and two Makita (Model 5007F) 7 1/4-inch Circular Saws in areas with standing water and mud which exposed employees to possible electric shock hazards.

(a) Rear side, new residential construction located at 748 Manor Drive, Smyrna, GA 30082: Employee was exposed to falls of approximately 24-feet, 4-inches while using a ladder jack scaffold support ladder as a means of access the scaffold platform. The employer failed to provide an additional ladder to access the ladder jack scaffold working platform.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  (a) Rear side, new residential construction located at 748 Park Manor Drive, Smyrna, GA 30082: Ladder jack scaffold that was being used by employees to install Hardi-Plank siding was not inspected by a competent person. The scaffold was used over heights of 20-feet, supported on two ladders with damaged side rails, split and dented rungs, missing footpads, and resting on a settling foundation consisting of mud and standing water exposing employees to potential collapse and fall hazards.

(Model 610132) Extension Ladder used to support a ladder jack scaffold platform had multiple damaged rungs and bent side rails and was not removed from service. Employee worked at heights of approximately 24-feet, 4-inches on a ladder jack scaffold platform supported by this damaged ladder.  (b) Rear side, new residential construction located at 748 Park Manor Drive, Smyrna, GA 30082: The 32' Green Bull Inc. (Model 608132) Extension Ladder used to support a ladder jack scaffold platform had damaged rungs, bent side rails and missing footpads on the base section. Employee worked at heights of approximately 24-feet, 4-inches on a ladder jack scaffold platform supported by this damaged ladder.
