29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: On July 30, 2012, at this construction site, an employee was workingon the top rail of aerial lift basket while doing welding operations at a height greater than sixteen feet along the northeast corner of the construction building. This condition exposed the employee to a fall hazard.
