Post Frame Building Under Construction: Employees performing various construction operations such as but not limited to installing the top section of the piggy back trusses where tools and materials were being handled were wearing tennis shoes in lieu of substantial work boots as observed on 7-23-12. 29 CFR 1926.404(b)(1)(ii):     Ground-fault circuit interrupters. All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.    a. Honda EM 6500 SX Generator: There was no ground fault circuit interrupter protection for employees that were using tools such as but not limited to the Makita circula saw that was being energized by the generator as determined on 7-23-12. Haulotte Scissor Lift: Employees were exposed to fall hazards estimated to be 18 feet as they stood on the mid rail and straddled the top rail of the guard rail system of the lift while installing plywood as determined on 7-23-12.    b. Haulotte Scissor Lift: Employees were exposed to a fall of approximately 14 feet due to the mid rail at the access area of the lift was tied up to the top rail resulting in an opening 28 inches wide by 42 inches tall making the guard rail system incomplete as determined on 7-23-12. 29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. Roof of the Post Frame Wood Building Under Construction: Employees were exposed to falls ranging in height from approximately 13 feet down between the 22 1/2 inch spaces between the trusses to the decked area below and also approximately 18 feet from the eave of the roof to the ground while installing the piggy back sections of the wooden trusses on the 8 in 12 sloped roof as observed on 7-23-12.    b.  Decked Area Inside the Trusses: Employees were exposed to falls of approximately 18 feet while walking / working from the decked area of the trusses due to the opening between the uprights of the trusses had a distance of approximately 22 1/2 inches where employees could fall as determined on 7-23-12.
