29 CFR 1910.23(c)(1):     Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    a) Production Area Press #21 - Employees used a 6' 6" high platform to service the machine but the guardrails on the machine was not continuous, thereby exposing the employees to a fall hazard, observed on or about 7-16-12.    Abatement cetification required.

29 CFR 1910.23(a)(2):     Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person can not walk directly into the opening:    a) Production Area Press #6 - Employees used a 6'8" platform to service the machine but the opening used to access the work platform from the ladder did not have a standard railing or other acceptable means to guard the opening, thereby exposing the employees to a fall hazard, observed on or about 7-16-12.    Abatement certification required.

29 CFR 1910.27(b)(1)(iii):     The minimum clear length of rungs or cleats was not 16 inches.    a) Ladder Accessing the Resin Silos From the Production Area Roof - Employees used a ladder on resin silo #4 to access the top of the 22 foot high resin silos and that ladder had rungs that were only 12 1/4 inches in length thereby exposing the employees to a fall hazard, observed on or about 7-16-12. 29 CFR 1910.23(a)(9):     Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment or walls) was not protected by a cover that leaves no openings more than 1 inch wide.  The cover shall be securly held in palce to prevent tools or materials from falling through:    a) Extruder Machine #2 - Employees walked past this end of this machine where a 9 foot by 12 1/2 inch opening under the edge of the machine did not have a cover, observed on or about 7-16-12.    Abatement certification required.

29 CFR 1910.147(c)(6)(ii):     The employer did not certify that periodic inspections of the energy control procedures had been performed:    (a) Production Area - The employees use Lock Out Tag Out procedures when working on equipment throughout the facility but the periodic inspections of those procedures being used on machines such as exruders and printers were not certified as of the time of the inpsection, reported on or about 7-16-12.
