29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about July 31, 2012, on top the scaffold in the front of the main building under construction, the employer allowed two employees to be exposed to an open end of the scaffold which did not have guardrails enclosing the end. The scaffold working platform was located 10 feet, 7 inches above grade. The employee was exposed to a fall from this height.
