29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: a) Lassiter Plastering Co. Inc. on or about July 30, 2012, at the address of 401 Lake Rd. Elyria, OH: The employer failed to ensure employees working from an aerial lift were adequately protected from falls. Employees working from the lift were not clipped off to the basket while the platform was elevated. The employees were exposed to an approximately 14 ft fall to the ground below.
