29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:    A written respiratory protection program was not developed or implemented to include: medical evaluations; procedures for cleaning, storage, maintenance, and use of the respirators, as well as, training the employees on these procedures. An employee wearing a half-facepiece respirator was exposed to hazards associated with respirator use while priming parts.

29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter:    An employee was exposed to respiratory hazards in that a fit test was not provided prior to initial use and annually thereafter. Employee wore a half-facepiece respirator while applying primers.

29 CFR 1910.1026(d)(1): Each employer who has a workplace or work operation covered by this section shall determine the 8-hour TWA exposure for each employee exposed to chromium (VI). This determination shall be made in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section:    An initial assessment of welders' 8-hour time-weighted exposure to hexavalent chromium was not conducted for stainless steel welding.
