At the job site, 2001 West Grand Ave, Salina, Kansas, machine operators were operating the following machines without point of operating guarding in place. Employees were exposed to ampuation and caught-by hazards. a. The MORI SEIKE LATHE was not equipped with a chuck guard to cover the rotating chuck. b. The AMADA Brake ID# HDS 1303, was not equipped with point of operating guarding.

Employees were exposed to struck-by hazards.
