29 CFR 1926.602(a)(3)(i): No employer shall move or cause to be moved construction equipment or vehicles upon any access roadway or grade unless the access roadway or grade is constructed and maintained to accommodate safely the movement of the equipment and vehicles involved: a/ Bayshore Recycling/Keasbey NJ Site -Yard: Concrete 1/8 of an inch in size, stock piled in (reversed cone shape) excess of 30 ft. in height was not secured, leveled off and/or other approved means provided; to prevent it from collapsing. Observed on/or about July 31, 2012.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): a/ Bayshore Recycling/Keasbey NJ Site: The lockout/Tagout program provided by the employer was not site specific, in that, there were no specific steps to de-energize the equipment located on site. Machinery includes, but not limited to, Crushing and Screening machines. Observed on/or about July 31, 2012.
