On our about July 27, 2012, Material handling operations such as loading the Mazak loading tower with steel plates required employees to use a powered industrial vehicle. The steel plates were not being placed as far as possible on the load engaging means and the mast was not carefully tilted backwards to keep the load stable.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
