29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about July 30, 2012, on the east side of the church, an employee was on the roof picking up screws, trash, etc. that had been left behind when the roof was replaced. The employee was exposed to fall hazards of approximately 20 feet to the ground below.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. (a) On or about July 30, 2012, on the east side of the building, the Dynalift DL 11-55 telescopic handler was left unattended and the load engaging means were not fully lowered, exposing the employee working on the roof to the hazard of being struck by the platform in the event of hydraulic failure.
