29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: a) Lemac Packaging Inc. dba Graphic Concepts, Inc / 645 West 32nd Street, Erie, PA: On or about August 2, 2012 an extension cord was in use in the Screen Department. The extension cord outer jacket was pulling away from the cable clamp at both the plug end and the connector end.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Lemac Packaging Inc. dba Graphic Concepts, Inc / 645 West 32nd Street, Erie, PA: On or about August 2, 2012 the eyewash station in the screen clean area was inspected monthly instead of weekly. Chemicals in the area used as part of the screen clean process includes, but is not limited to materials with a pH higher than 11 and pH of less than 4. The chemicals include, but are not limited to LSR-30, ER/30s, GR-6V and Liquid Haze 950.
