Lemac Packaging, Inc. dba Graphic Concepts, Production areas, on or about January 17th, 2019, and times prior thereto: Employees worked with and around hazardous substances including, but not limited to, various inks (3M brand and Duraflex-Polymeric brand) and IW-107 screen wash during screen printing and cleaning operations. A written hazard communication program had not been developed and implemented.
