29 CFR 1910.23(a)(8):  Every floor hole into which persons can accidentally walk were not guarded:    On 08/02/2012, floor holes were not guarded by standard railing with standard toeboards on all exposed sides or by floor hole covers of standard strength and construction:    a)  On the second floor in the pattern storage area, employees were exposed to an unprotected floor hole approximately 8 inches wide by 34 inches long adjacent to the sand silos. The floor hole cover was not secured and could be easily moved to expose the floor hole.    b)  On the second floor in the patter storage area by the far wall from the stairs, a cut-out around a pipe approximately 12x16 inches wide was covered with a piece of sheet metal that was not of standard strength.    This employer has been previously cited for this practice that includes:    Performance Industrial Products LLC. was previously cited for a violations of this Occupational Safety and Health Standard 1910.23(a)(8) which was contained in OSHA Inspection Number 91004 Citation 02, Item 001 and was affirmed as a Final Order on 04/27/2012, with respect to a workplace at 512 N. Mill Street, Weyauwega, WI.    Performance Industrial Products LLC. was previously cited for a violations of this Occupational Safety and Health Standard 1910.23(a)(8) which was contained in OSHA Inspection Number 85912 Citation 01, Item 002 and was affirmed as a Final Order on 04/26/2010, with respect to a workplace at 512 N. Mill Street, Weyauwega, WI.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
