29 CFR 1910.303(f)(1):  Each disconnecting means required by Subpart S of Part 1910 for motors and appliances was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    a)  On 08/02/2012, the labels and markings on a control box for the ventilation cabinet of the swing grinder located in the casting cleaning department's southeast corner were not legible due to accumulations of metal swarth which covered the control box's labels and buttons.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.303(g)(1)(i)(A):  For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    a)  On 08/02/2012, the depths in the direction of access for electrical boxes for the small tube cut-off saw, which were located behind the small tube cut-off saw, were less than 3 feet.     ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.23(a)(8):  Every floor hole into which persons can accidentally walk were not guarded:    On 08/02/2012, floor holes were not guarded by standard railing with standard toeboards on all exposed sides or by floor hole covers of standard strength and construction:    a)  On the second floor in the pattern storage area, employees were exposed to an unprotected floor hole approximately 8 inches wide by 34 inches long adjacent to the sand silos. The floor hole cover was not secured and could be easily moved to expose the floor hole.    b)  On the second floor in the patter storage area by the far wall from the stairs, a cut-out around a pipe approximately 12x16 inches wide was covered with a piece of sheet metal that was not of standard strength.    This employer has been previously cited for this practice that includes:    Performance Industrial Products LLC. was previously cited for a violations of this Occupational Safety and Health Standard 1910.23(a)(8) which was contained in OSHA Inspection Number 91004 Citation 02, Item 001 and was affirmed as a Final Order on 04/27/2012, with respect to a workplace at 512 N. Mill Street, Weyauwega, WI.    Performance Industrial Products LLC. was previously cited for a violations of this Occupational Safety and Health Standard 1910.23(a)(8) which was contained in OSHA Inspection Number 85912 Citation 01, Item 002 and was affirmed as a Final Order on 04/26/2010, with respect to a workplace at 512 N. Mill Street, Weyauwega, WI.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
