29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) On or about 7/31/12 inside building under renovation at 175 Jefferson Ave., Buffalo, NY: Employees were exposed to the potential of stepping into a shallow, narrow, rectangular floor hole which was left uncovered. This hole was approx. in the center of the building and the length of the hole ran in the north-south direction. Electrical tools, supplies & equipment were stored near the hole which was approx. 15 in. wide x 20 ft. long x 12 in. deep. NO ABATEMENT CERTIFICATION REQUIRED
