29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or about August 3, 2012, employees at DuPage Welding, Inc. were required, by the employer, to wear half-mask respirators. The employer did not establish and implement a written respiratory protection program for these employees. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

The employer did not develop, implement, or maintain a written hazard communication program at the workplace. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
