29 CFR 1910.332(b)(1):     Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    Baymont Inn & Suites, located at 2600 Lauren Lane, Northwood, OH 43619: On or about August 3, 2012, the employer did not ensure that maintenance employee was provided with training on electrical safety-related work practices such as, but not limited to, using test equipment to verify that the circuit elements and equipment parts of recessed light housings were deenergized prior to its replacement, in order to protect the employee from electrical hazards. The maintenance employee replaced old light housings with IC Recessed Housings (manufactured by Patriot Lighting) without conducting proper verification that circuit elements and equipment components were deenergized.    Baymont Inn & Suites was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.332(b)(1)), which was contained in OSHA Inspection # 313786725, Citation number 01, Item number 001 (b), and was affirmed as a final order on August 31, 2011, with respect to a workplace located at 2600 Lauren Lane, Northwood, OH 43619.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.333(b)(2)(iv)(B):     Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that:    Baymont Inn & Suites, located at 2600 Lauren Lane, Northwood, OH 43619: On or about August 3, 2012, the employer failed to ensure that a qualified person used test equipment to test the circuit elements and electrical parts of recessed light housings in order to ensure that the circuit elements and equipment parts were deenergized. The maintenance employee replaced old light housings with IC Recessed Housings (manufactured by Patriot Lighting), and was exposed to electrical hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
