29 CFR 1926.451(c)(2):   Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    The employer does not provide the scaffold with base plates, mud sills or other adequate firm foundation.  This violation was observed on or about August 2, 2012, on the south side of a newly built house where employees were exposed to a fall hazard when required to work on the platform at a height of 13 feet without base plates, mud sills or firm foundation.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that scaffold poles, legs, posts or uprights bear on base plates, mud sills or other adequate firm foundation.

29 CFR 1926.451(g)(4)(i):   Guardrail systems were not installed along all open sides and ends of platforms:    The employer does not install guard rails on the scaffold.  This violation was observed on or about August 2, 2012, on the south side of a newly built house where employees were exposed to a fall hazard when required to nail siding from a platform of 13 feet without a guard rail system.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guardrail systems are install along open sides and ends of platforms.

29 CFR 1926.452(j)(2):   Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    The employer does not require that pump jack scaffold be secured to the structure by rigid triangular bracing at the bottom, top and other points as necessary.  This violation was observed on or about August 2, 2012 in the residential housing area where employees were exposed to a fall hazard when required to work from the pump jack scaffold without it being secured to the structure at the top, bottom and other points as necessary with triangular bracing.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees secure poles to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.
